How to Prepare for Your In-take Appointment

      Health Assessment:

  • Complete the Lifestyle & Medical History Form and provide it to your Health Care Professional if it was not already provided.

  • Prepare a list of prescription medications and recent lab work, if applicable, as well as any supplements and labels of unique foods that you eat regularly or that you want included in your nutrition plan. (e.g.: Cultural or religious-based food items such as Kosher or Halal)

    Fitness Evaluation

  • Complete the ParQPlus Form and provide it to your Health Care Professional.

     Genetic Gene Test

  • If you have not already sent in your saliva sample, please do so right away.

  • DO NOT eat, drink, smoke or chew gum for 30 minutes before giving saliva sample.

  • Complete the Genetic Test Consent Form and provide it to your Health Care Professional.

More Information

  • Contact your insurance company to ask if your policy covers medical nutrition therapy by a Registered Dietitian for your diagnosis or health concern. (We can verify coverage for you)

  • Appointments take up to 1 hour to complete.


Save time by printing and completing the consent forms in advance of your appointment.


If you are unable to print the consent forms in advance, your Health Care Professional will provide you with blank copies and will help you complete them if needed.

Saliva Collection Kit

  • A saliva collection kit will be sent to your home. Please complete the instructions for returning your saliva sample to our lab.

  • If you wish to provide your saliva sample to our Health Care Team Member, please be sure to bring your saliva collection kit with you to your appointment.
